Gold RN provides a vast array of services such as developing and implementing a plan of care for you in the comfort of your own home, hospital, or facility and managing all the moving pieces that come with needing care. Gold RN will be your one point of contact that handles all other aspects so you do not have to.

IMPLEMENTING A CARE PLAN JUST FOR YOU

Gold RN is here to assist you in countless ways. Some examples include working closely with your entire treatment team to ensure efficient and prompt collaboration and execution of your plan of care. This may involve managing and administering home medications, managing and accompanying you to doctor appointments or treatments, assisting with and facilitating admissions and discharges to the hospital as well as transitioning to rehab, palliative care, hospice, or other placement. We also work with your family, friends, colleagues and your household to tend to all aspects of a complicated time in your life.

We create and implement in home care systems for organization, efficiency and clarity that can involve medication management, wound care education, supply set up, appointment maintenance and reminders, and client care logs or organization of doctors with their specialities, treatment they provide, contact information, and medications prescribed, in an easy to access format for you and/or your loved ones.

We brainstorm ways to make your life easier with creative and unique systems that work with your strengths and that you will find useful. We also create them to be user friendly and easily accessible should you need to share them with others.

We take the time to educate you on each and every aspect of your care so you are making informed decisions, relay any questions, concerns and desires to your medical team and also serve as a support and guide when it's time to make difficult health related choices, including short or long term placement, palliative or end of life care and researching options for you and your loved ones.
